6. Conduct: I full understand that all participants are to abide by and accept all rules and requirements governing conduct during the program. Rules and expected conduct will be thoroughly discussed with the participants on their first day. Any participant determined to be in violation will be sent home at their own or their parent/guardian's expense.
7. Waiver of Claims for Liability: The Ronald Reagan Student Leadership Program will be held entirely on the premises of the Ronald Reagan Presidential Library and Museum. Participants and their parents/guardians hereby release and discharge, without limitation, the Foundation and the Foundation's staff, trustees, vendors, and other agents (including the National Archives and Records Administration and their staff at the Library), from any and all claims, demands, and causes of action that participants may have against them by reason of attendance at this program, including but not limited to, any personal injuries or property damage.
8. Emergency Care Consent: In the event of significant illness or injury to participant, should program staff not be able to reach a parent/guardian or other emergency contact, I hereby consent to whatever transportation, x-ray, examination, anesthetic, medical, dental, or surgical diagnosis or treatment and hospital care from a licensed physician as deemed necessary for the safety and welfare of my child. It is understood that the resulting expenses will be the responsibility of the child's parent(s)/guardian(s) as listed on this form.
9. Transportation: Local students are responsible for their own transportation to/from the Ronald Reagan Presidential Library and Museum in Simi Valley, California. If your child will be driving themselves, it is the responsibility of the student to arrive on time each morning. Parents picking their child up from the designated meeting location must arrive at least 15 minutes prior to the stated return time. The Ronald Reagan Presidential Foundation and Institute assumes no responsibility for students left unattended due to parent tardiness. In the event of an emergency, I hereby consent to whatever transportation (Foundation vehicles, private vehicles, commercial vehicles, walking, etc.) is deemed necessary for the safety and welfare of my child.
