Ronald Reagan Student Leadership Program
Thank you for your interest in the 2020 Ronald Reagan Student Leadership Program. To streamline the registration process, you will need the following:

•    Student email address (please use a personal email, not a school email address)
•    Parent/Guardian contact information (email address, mailing address, phone number, etc.)
•    School information
•    Medical, dietary restrictions, and emergency contact information

You may save and resume your registration at any time. You will be asked to register for a preferred week and submit a non-refundable deposit.

For day camp, a non-refundable deposit of $75 for early registration and a $125 deposit for regular registration is required.

For residential camp, a non-refundable deposit of $200 for early registration and a $350 deposit for regular registration is required.
Student Information: We require the following information for purposes of understanding our population better and helping our staff use the most respectful language when addressing your child. Please help us serve you better by selecting the best answers to these questions.

Parent/Guardian Release
1.  Permission to Participate: If accepted, I hereby give persission for my child to participate in the Ronald Reagan Student Leadership Program. For day camp, I understand that the student must be available for all days of the session to which they are accepted, and must be available from 9:00 am – 3:30 pm. For residential camp, I understand that the student must be available for all days of the session to which they are accepted, and must be available from Sunday 3:00 pm – Saturday 12:00 pm.





***Please note – Due to the volume of food produced by the catering department for special events, the Reagan Café and the Reagan Pub, the Foundation cannot guarantee a peanut-free environment. In some causes, the student may be responsible for bringing their own meal.***

If yes, the Ronald Reagan Presidential Foundation and Institute will require a signed Distribution of Medication Authorization form.

6. Conduct: I full understand that all participants are to abide by and accept all rules and requirements governing conduct during the program. Rules and expected conduct will be thoroughly discussed with the participants on their first day. Any participant determined to be in violation will be sent home at their own or their parent/guardian's expense.
7. Waiver of Claims for Liability: Day camp will be held entirely on the premises of the Ronald Reagan Presidential Library and Museum. Residential camp will be held on the premises of the Ronald Reagan Presidential Library and Museum as well as in California Lutheran University. Participants and their parents/guardians hereby release and discharge, without limitation, the Foundation and the Foundation's staff, trustees, vendors, other agents (including the National Archives and Records Administration and their staff at the Library), and/or California Lutheran University and their staff from any and all claims, demands, and causes of action that participants may have against them by reason of attendance at this program, including during travel to and from the Library or CLU Campus, including but not limited to, any personal injuries or property damage.
8. Emergency Care Consent: In the event of significant illness or injury to participant, should program staff not be able to reach a parent/guardian or other emergency contact, I hereby consent to whatever transportation, x-ray, examination, anesthetic, medical, dental, or surgical diagnosis or treatment and hospital care from a licensed physician as deemed necessary for the safety and welfare of my child. It is understood that the resulting expenses will be the responsibility of the child's parent(s)/guardian(s) as listed on this form.
9. Transportation: Local students are responsible for their own transportation to/from the Ronald Reagan Presidential Library in Simi Valley, California. If your child is relying on bus transportation to/from the museum, it is the responsibility of the student to arrive 15 minutes prior to the stated return time. Parents picking their child up from the designated meeting location must arrive at least 15 minutes proor to the stated return time. The Ronald Reagan Presidential Foundation assumes no responsibility for students left unattended due to parent tardiness.
10. Media Release: I hereby grant the Ronald Reagan Presidential Foundation the right to obtain and/or use my child's photograph, digitized image, video image and/or voice for educational and informational purposes in conjunction with the Ronald Reagan Student Leadership Program. I understand that all images, productions and content, therein, become the exclusive property of the Ronald Reagan Presidential Foundation. I understand that there will be no compensation for the use of any photograph, digitized image, video image and/or voice taken from within the Ronald Reagan Presidential Library and Museum. It is the Ronald Reagan Presidential Foundation's policy that students in photographs are not identified by name.
